Pennsylvania requires a federal background check for all employees or volunteers. In many circumstances, you can submit a signed child safety affidavit instead of the FBI Fingerprint History. Use the following table to determine if you need to complete an FBI Fingerprint History or a Child Safety Affidavit.

Child Safety Affidavit
If you are able to complete the Child Safety Affidavit, you may do so by downloading the appropriate PDF, sign it and submit it via GotSport.
- If you are 18 years or older, and have continuously lived in Pennsylvania for the past 10 years, use this version of the Child Safety Affidavit.
- If you are under the age of 18, you and a parent, or legal guardian, will need to use this version of the Child Safety Affidavit for Minors.
- Upload your signed child safety affidavit using GotSport
FBI Fingerprint History
If you are 18 years or older, and have not lived in in Pennsylvania for more than 10 years, you will need to complete an FBI Fingerprint History. Using the following steps to complete the process:
- Email risk@perkvalleysoccer.org to let us know that you need to complete the FBI Fingerprint History
- Go to https://uenroll.identogo.com/workflows/1KG738
- If prompted, enter 1KG738 as your code.
- Fill out the information on the website to schedule an appointment (appointments are not required, but strongly encouraged)
- Submit a copy of your receipt to risk@perkvalleysoccer.org and we will reimburse you for the cost.
- Go to your appointment and don’t forget to take your required forms of identification
- In a few days you should receive your results electronically
- Upload the results to GotSport.
