Each year, coaches must electronically sign the EPYSA Risk Management Acknowledgment Form. It's a series of statements indicating that you understand and agree to abide by the Eastern Pennsylvania Youth Soccer Association's (PVSC's sanctioning body) risk management process.

To complete the form:

1. Log into https://system.gotsport.com
2. On your Dashboard locate the item labeled "Eastern Pennsylvania Youth Soccer Risk Management" under your requirements
3. Click on the item
4. Click on "Submit New Report"
5. Read each statement and check the checkbox indicating that you have read and agree to abide by the statement
6. Add your digital signature
7. Click"Save"
